Our branches provide services to patients in their homes. This can include nursing homes and assisted living facilities. Short term inpatient care for pain and symptom management, or to provide a period of respite for the family is also available.  All services related to the terminal illness are covered fully by Medicare & Medicaid, and in some cases TRICARE Veterans Benefits.

Hospice care is not provided by one person alone. It is provided through an interdisciplinary, medically-directed team.

All of our branches specialize in hospice care. It's all we do, so our team is made up of highly qualified, trained professionals with years of hospice experience. All our team members are skilled in providing quality comfort care, in close coordination with the patient's personal, attending physician and the hospice's Medical Director.

Hospice Medical Director

The Hospice Medical Director consults with the patient's attending physician and oversees the medical aspects of our branches program and services. The Hospice Medical Director is available to answer patient/family questions regarding hospice care. Coordination of care with the patient's attending physician occurs on a regular basis, as well as in times of crisis.

Home Health Aide

The Home Health Aide provides for the personal care of the patient. This includes bathing and skin care, feeding and dressing. The Home Health Aide will often prepare meals and do light housekeeping. Often times, the hospice patient and family develop a supportive bond with their Home Health Aide; one that fosters open communication and emotional trust and support.

Clinical Manager

The Clinical Manager is a professional, registered nurse responsible for assuring that services are delivered in accordance with the plan of care. The Clinical Manager works with the Medical Director to bring all the necessary medical services together to develop the best plan of care for the patient.

Social Worker

The Social Worker addresses the psychosocial issues surrounding the patient and family. The Social Worker will also assist the patient and family in accessing community resources. The Social Worker's job is to listen, counsel and provide support concerning patient health, social and financial issues such as insurance, living wills and advance directives.

 

Case Manager

The Case Manager is a professional, registered nurse responsible for carrying out skilled day-to-day care, including pain and symptom management. The Case Manager leads the coordination of medications, medical equipment and supplies and makes regular scheduled and un-scheduled visits. They keep the attending physician and the Medical Director informed of the patient's condition. They are on the front-line, serving the patient's needs by implementing the latest pain and symptom management techniques and unique therapies.

Spiritual Counselor

The Spiritual Counselors are available to answer questions, counsel and provide emotional and spiritual support to the patient and family. The Spiritual Counselor also often acts as a liaison between the family and personal clergy. Spiritual concerns can come from fear of the unknown, worry about leaving loved ones behind and overall anxiety and grief. Our Spiritual Counselors are skilled at addressing these concerns personally and confidentially with patients and family members.

Volunteers

Volunteers are trained to provide companionship and support to patients and family members. They help out in a variety of ways, such as running errands and providing companionship. They may also stay with the patient while family members take a much needed break.

Bereavement Team

The Bereavement Team supports the family for 13 months during the grieving process. Individual counseling, group counseling and other supportive services are provided to assist families through the very normal and sometimes unpredictable grief period.
